The downloaded application packet (see above) will include the following forms.
MAIN APPLICATION
The Main Application for Admission contains a section for the student and a section for the Parent(s). Please fill out this form and return it to the Admission Office as soon as possible.

TEACHER RECOMMENDATION FORMS
Please give the Teacher Recommendations to the current Math and English teachers. You and your parents should sign the Waiver of Access before submitting these forms to your teachers. When you give the forms to the teachers, please include an envelope addressed to the Admission Office so that they may mail the forms directly to Baylor.

PERSONAL RECOMMENDATION FORM
Please give the Personal Confidential Recommendation to the adult of your choice. Attach a stamped envelope for them to mail the forms directly back to Baylor. It can be a teacher or mentor who knows you outside of the classroom, but please be sure that this person has frequent contact with you. Please have the recommendation forwarded to us as it becomes available.

SCHOOL REPORT AND TRANSCRIPTS FORM
Please give the school report and transcript form to your Principal, School Head, or Guidance Counselor. The transcript should include all grades for the two years prior to your application, as well as from the first semester of the current school year. Please mail the transcript and school report to the Admission Office after the first semester of the current school year. You may request that the school mail this information directly to Baylor’s Admission Office.

Students already in high school should provide a transcript with all courses and grades taken for high school credit.

SSAT and ISEE TEST
To register online for the required SSAT or ISEE test, visit their website at www.ssat.org or www.erbtest.org. Please notify us of your scheduled test date, contacting Cindy Clark at cindy_clark@baylorschool.org or (423) 267-5902.

The admission program at Baylor School operates under a rolling admission policy. We require school transcripts, teacher recommendations, testing, an interview, and a writing sample from all of our applicants. Decisions are made on a weekly basis while spaces are available.
